REQUEST FOR STATEMENT OF QUALIFICATIONS (RFQ)

CITY OF BLOOMINGTON, ILLINOIS

The Request for Statement of Qualifications (RFQ) will be extended for one (1) week due to an administrative oversight.  Firms who have already submitted RFQs – NO ACTION IS REQUIRED.  Any firm who has not submitted a RFQ may do so and RFQs need to be received at the office of the City Clerk, City Hall, 109 East Olive Street, Bloomington, Illinois 61701, until Thursday, March 28, 2019 at 10:00 am Central Time for the following:

Extended RFQ # 2019-34

RFQ documents are to be obtained from the City of Bloomington Procurement Services by sending an email to procurement@cityblm.org.  Respondents must provide their complete name, company name, street address, telephone number, fax number and their email address.  RFQ documents may also be obtained at the office of the City Clerk, Monday through Friday between the hours of 8:00 a.m. and 5:00 p.m. Central time.  Only vendors recorded with the City, as a bona fide plan holder, are eligible for the contract award. 

You may not respond with a faxed or electronic submission.  All statements must be submitted in a sealed envelope, with original signatures.  One (1) original hard copy (includes all three listed items below) and five (5) electronic copies on USB Flash Drives or Compact Discs of the complete statement as listed below.  A complete statement is:

  1. One set of the City documents

  2. Cover page for each category submission

  3. Maximum of forty (40) pages for each statement of qualification category

The City of Bloomington reserves the right to reject any and all statements and to waive technicalities.  The award will be based on the criteria for selection contained within this document.
