REQUEST FOR STATEMENT OF QUALIFICATIONS (RFQ)

CITY OF BLOOMINGTON, ILLINOIS

Statement of Qualifications (RFQ) will be received at the office of the City Clerk, City Hall, 109 East Olive Street, Bloomington, Illinois 61701 until Tuesday, June 2, 2020 @ 2:00 p.m. Central Time for the following:

POTABLE WATER RISK & RESILIENCE ASSESSMENT

& EMERGENCY RESPONSE PLAN

RFQ documents are to be obtained from the City of Bloomington Procurement Division by sending an email to procurement@cityblm.org.  Respondents must provide their complete name, company name, street address, telephone number, fax number and their email address.  Only vendors recorded with the City, as a bona fide plan holder, are eligible for the contract award. 

You may not respond with a faxed or electronic submission.  All statements must be submitted in a sealed envelope, with original signatures. One (1) original hard copy, including City RFQ documents, and an electronic copy on USB Flash Drive or Compact Disc is required.  In response to the unprecedented situation and to limit the community spread of COVID-19, the City of Bloomington strongly encourages that all vendors send their sealed bid/proposal submissions by USPS or Courier to the City of Bloomington, Attn: Office of the City Clerk-Bid Submissions, 109 East Olive Street, Bloomington, IL, 61701

The City of Bloomington reserves the right to reject any and all statements and to waive technicalities.  The award will be based on the criteria for selection contained within this document.  Submissions will be considered valid for sixty days from the due date of June 2, 2020.
